STEP 1: Download the form to your local computer / drive
STEP 2: Always use Adobe Reader DC or Adobe Acrobat DC to open and fill in SFA forms
FREE Adobe Reader DC is available for download.
STEP 3: Always save the filled in PDF form with a proper file name such as following: “FileName_StudentName_ID”, before sending/submitting the form to Student Financial Aid office.
STEP 4: Use Secure Document Upload function in your MyView Student Center to upload and submit your forms to Student Financial Aid office.
If you decide to email the form, please make sure your Student Name and ID are included in the email.
(DO NOT EMAIL a form with your sensitive information on it. Use Secure Document Upload function instead.)
